Please note that this form requires the signature of both student and parent/s or legal guardian regardless of the
student’s age. In particular, if a student is considered a dependent for federal or state income tax purposes or for
financial aid, the parents waiver portion of this form must be signed and returned prior to participation. If the
student is financially and legally independent, you should send us a letter to that effect and request an alternate
form. Please make a copy for your records.
